Lively dragon waivers.

First waiver is a personal waiver consisting of three pages.

• First page is an acknowledgment of risk form
• Second page is a standard waiver form
• Third page is a medical information form ( this is at the paddlers discretion, it is for your own benefit if you have a heart condition, epilepsy or anything that you think your captain, team manager , Lively Dragon or a Emergency Response Team  should be aware of)
• Fourth form is a Team waiver form that must be signed by all members of your team.
